If your Web Profile includes more than one of your accounts, pay special attention to your choice of Company Code and Processing Office. Once selected, these values cannot be changed.
Your Reference Number
Any "subject" or reference number that you would like to use to identify the application. A valid value is up to 20 characters in length. (Mandatory entry.)
Processing Office
The administrative district office responsible for your account and licence application. The district office is identified by the first two digits of your account number. Select an office from the drop-down list. The list contains offices associated with the accounts in your Web Profile.
Company Code
A numeric code that identifies an account holder to Industry Canada. Your account number is a two-digit distict office code followed by this seven-digit company code. The value of this field is set when you first create the covering letter, and cannot be updated.
Account Information
After you create and save your covering letter, shows much of your account information as stored in the Assignment and Licensing System (ALS) database for the company code and processing office that you selected when you created the covering letter. If you notice where we should update our records, please contact your district office. (This information does not form part of your application.) (Account information is not displayed when editing the covering letter.)
Contact Name
The full name of the person responsible for this application. (Mandatory entry.)
Phone Number (999) 999-9999
The telephone number of the person responsible for this application. A valid phone number is in the format (999)999-9999, and includes the area code. (Mandatory entry.)
Ext.
The telephone extension number of the applicant. The extension number must be a numeric value.
E-Mail Address
The e-mail address of the applicant.
System Description
Description of the new system and/or the changes to authorized system. Although this is an optional field, we recommend that you complete it to provide us with a better understanding of the design of any system with multiple stations.
Nature of Service (taxi dispatching, construction, etc.)
Describe the business or other activity supported by the system, i.e., the purpose of your system. (Mandatory entry.)
In-Service Date
Date that the applicant proposes to put the system in service. The format is YYYY-MM-DD.
Is this an application for a short-term licence ...?
Indicate if you are applying for a short term licence. A short term licence is valid only for one to three periods of thirty days each. Use the In-Service Date to provide the data that you will commence operation, and the Special Requirements area to indicate if more than one term is required.
Is an interim authority requested?
Indicates that the applicant requires interim authority to use a temporary frequency while the application is being processed and international coordination is being conducted. You should justify your need for a temporary frequency in the Special Requirements field.
Does the application being made include a frequency authorized to a third party...?
Indicates whether the the applicant is the primary licencee of the frequencies requested, or will be using a frequency authorized to a third party. Enables validation of other third-party information fields. (If more than one third-party authorization is required, additional information can be provided in the Special Requirements field.) You cannot enter values in the other third-party information fields unless you select Yes in response to this question.
Have you consulted the third party...?
If this covering letter is for a third-party application, indicates whether the third-party licencee has been consulted and approved the proposed use of their authorized frequency.

Station Location or Area of Operation
Municipality, site name, or area of operation of the third party station. Mandatory for third party applications.
Tx Frequency Requested (MHz)
Transmission (Tx) frequency licenced by the third party to be used by the applicant. Must be between 0.1000000 and 960.000000.
Rx Frequency Requested (MHz)
Reception (Rx) frequency licenced by the third party to be used by the applicant. Must be between 0.1000000 and 960.000000.
Number of Mobile Stations (if applicable)
Number of mobile stations that the client intends to use on the third party's frequency. Mandatory for mobile-to-mobile third party applications.
Third Party's Account Number (if known)
The account number (district office and company code) of the third party, if known.
Third Party Licensee Name
Name of the third party, i.e., the business or person authorized by Industry Canada to use the frequency you would will be sharing n your area of operation. Mandatory for third party applications.
Contact
Name of the person who represents the third party and may be contacted regarding your application.
Telephone Number (999)999-9999
Telephone number of the person who represents the third party.
Special Requirements
Describes any special requirements for the system. May also be used to include additional third party information for systems in communication with more than one third party. Specifically, you should indicate any unusual or outstanding features of your system. For example:
